Please fill out the request form completely. Do not submit more than one form for the same place. Transcript requests are $15.00 per request. REQUESTS FROM SCHOOL DISTRICTS, SUPOENAS, ATTORNEYS AND DEPARTMENT OF CORRECTIONS ARE NOT REQUIRED TO PAY THE FEE.

NOTE: Any 3rd Party Requester needs to mail a 3rd Party Supplemental Authorization for Release of Records Form to Wayne RESA – attention: Transcripts – 33500 Van Born Road, Wayne, MI 48184.
